This policy describes UCSF’s academic and behavioral expectations for our students as well as procedures if students do not abide by those expectations.

Report an Incident

To share information regarding an incident involving a UCSF student or student organization, use the student conduct reporting form to file a report. Anonymous reports may restrict the University’s ability to initiate the conduct process. Include the following information in your report:

  • Name of alleged student(s) and/or student organization(s)
  • Date, time, and location of alleged incident
  • Detailed statement of what you observed/heard/experienced
  • Names & contact information for any witnesses
  • Any additional pertinent information, such as screenshots, emails, documents, etc.

We appreciate you sharing your concerns and reporting incidents in order to maintain a safe and secure campus environment. While we may contact you for follow-up information regarding your report, it is important to note that privacy and confidentiality under the Family Educational Rights and Privacy Act (FERPA) may prohibit us from sharing the status or outcome of a conduct case with the reporting party.

Information shared about an incident may become a part of the student’s record, and the student has the right to see this documentation. If you are unsure about filing a report or what information to include, email [email protected] to set up a time for a consultation prior to submitting a report.
